Cervical cancer or cancer of the cervix is an abnormal growth of malignant (cancer) cells in the cervix. It is the second most common cancer in females and it is successfully curable in the early stages. Cervical cancer occurs most commonly between 40 and 55 years of age. The cervix is the opening of the uterus connected to the upper vagina. It is known as the neck of the uterus. The lower end of the cervix has an opening called the external os, and before childbirth it is about 5mm in diameter. It dilates to 10cm during labour to allow for childbirth and never regains its former shape, changing to a slit approximately 7mm long. Normally the cervix is pink in colour but turns bluish in pregnancy because of an increased blood supply. The surface layer of the cervix is made up of two different types of cells, flat cells called squamous cells and tall column-like gland cells called columnar cells. This produces a mucous membrane, but the mucous membrane of the cervix is smooth. The place where squamous and columnar cells meet is known as the transformation zone and this is the area of the cervix where cancer most commonly arises. The cancer cells may be present in the cervix for 4-10 years before becoming invasive, affecting the deeper tissues and giving rise to symptoms. Even when the cancer is invasive, there is an 80% chance of successful cure. Once it spreads through the pelvis to the vagina, uterus, bladder or rectum, Cervical Cancer is much more difficult to cure.
Causes
Although the exact causes of cervical cancer are unknown, several risk factors appear to be linked with the disease, such as -
- Infection HPV Virus - More than 90% of women with cancer of the cervix are infected with the Human Papilloma Virus (HPV). HPV is the single most important factor for cervical cancer. HPV usually causes warts in the genital area. The viruses are passed from one person to another during unprotected sex. Because HPV is spread mainly through sex, women who start having sex at a young age, who have multiple sexual partners, and whose partners have had many other partners are more likely to have HPV.
- Early sexual activity is one such factor. The cells lining the cervix do not fully mature until the age of 18 and this increases the risk of HPV that causes Cervical Cancer.
- Other Sexually transmitted diseases (also called STDs or STIs - sexually transmitted infections) including gonorrhea, chlamydia, syphilis or HIV/AIDS are also increases the risk of HPV that causes cervical cancer.
- Cigarette smokers are two to three times more likely than nonsmokers to develop cervical cancer
- Possible long-term (more than five years) use of oral contraceptives can also cause cervical cancer.
- An irregular screening history- Women who have not regularly had a Pap test (smear) are at increased risk of cervical cancer. In particular, many women over age 60 have not had regular Pap tests and are at increased risk.
Symptoms
Early cervical cancer generally produces no signs or symptoms. But, as the cancer grows, symptoms may include -
- Bleeding from your vagina, such as between menstrual cycles or after sexual intercourse
- Vaginal bleeding after the menopause
- An unpleasant vaginal discharge
- Discomfort during intercourse
- In advanced stages, it causes pelvic pain
- Vaginal leakage of urine and feces from a fistula, anorexia, weight loss, and anemia.
Anyone with these symptoms should see a doctor so that the doctor can treat any problem as early as possible. These symptoms are not sure signs of cervical cancer. There are many other conditions that can cause the same symptoms. An infection or other problem could also cause these symptoms.
Treatment
Treatment for cervical cancer depends on tumor size and location, disease stage and the patient's age and overall health. Cervical cancer most often treated with one or a combination of treatments: surgery, radiation, and/or chemotherapy. Cervical cancer is curable by removing or destroying the pre-cancerous tissue. If the cervical cancer in its early stages, a simple hysterectomy or a radical hysterectomy can be performed. But in the most advanced cases, the pelvic extenteration is used.
